Insurance is a contract between you and your insurance company. We are not a party to this contract, in most cases. We participate with several insurance companies, and as a courtesy to our patients; we electronically transmit your claims via our computer modem at the end of each business day. Although we may estimate what your insurance company may pay, it is the insurance company that makes the final determination of your eligibility. Remember that deductibles and insurance maximums apply in most cases. You agree to pay any portion of the charges not covered by insurance. Failure to submit proper insurance information, which this office accepts on the initial visit, will result in your account being self-pay until such information is provided to the staff.
Fees are determined in consideration of the practice's requirement to provide the very best care possible to our patients.
Payment in full is expected at the time of service. For services requiring multiple appointments, we require half of the payment at the first appointment, and the remaining balance due at the final appointment. In an effort to keep expenses down, we do not mail statements to patients. We require that all co-pays, deductibles, and non-covered expenses be paid at the time of the appointment. As a courtesy to you, we will bill these to a credit card kept on record in our office at the final insurance payment. You must sign an authorization to charge your credit card.
We accept Visa, MasterCard, Discover, and American Express. Financing is available through Care Credit.
